* If you have multiple family members who are patients, please do a separate submission for each patient.
3. Enter your credit card and billing information or use a linked smartphone payment method like Apple Pay.
4. Once you have made your payment, please send a text message to (205) 943-1197 or email receptionist@birminghamallergy.com with the patient’s name, date of birth, and payment amount, so we can apply it to your account correctly.
