New Patients

Visiting our office for the first time? You can decrease your wait time by completing our patient forms prior to your visit, available here.

 

If you would like a flu shot at your next visit*:

Please complete the flu vaccine consent form and authorization to pay benefits to the provider and bring them with you to your next appointment.
*For patients ages 4 and up only. Children must have had at least 1 flu shot previously. Medicaid patients should get a flu shot from their primary care doctor or pediatrician. We cannot administer flu shots to Medicaid patients at our office.

 

In the event of inclement weather or an inclement weather forecast:

Please contact us prior to your appointment so that we may advise you on our hours for the day.

 

Appointment cancellations and rescheduling – Cancellation Policy & No Show Fee:

We are happy to work with you to see you at a more convenient time; however, please note that all cancellations (including rescheduling) must be made at least 24 hours in advance.  If you are unable to come in the office for your appointment, please know that you have the option to convert your visit to a telemed. Failure to do so without proper notice, or if you do not show up for your scheduled appointment, will result in a $50 fee that will be assessed.

In other words, missed appointments (aka “No Show”) and appointments cancelled with less than 24-hour notice, will result in a fee of $50 charged to your account. The patient will be responsible for these charges, not the insurance companies, and payment will be due prior to your next appointment.

 

Copays:

All copays are due at the time of service.

 

Forms:

We are happy to complete forms for school, camp, etc. at your office visit at no charge. There is a fee for any forms completed by the physician outside of the scheduled office visit. Please call or email receptionist@birminghamallergy.com for more information.

 

COVID-19:

If you are coming into the office for any reason, you must follow these guidelines. (Please see instructions for arrival text messages at each office.) Appointments are available as both in-office visits and telemedicine virtual video appointments. We are accepting new patients with the option for telemedicine virtual appointments, also. Allergy shots will be given by appointment only until further notice. Thank you for your cooperation and understanding.
